Will it lay flat when folded over or behind?

Hi Laura, Thanks for letting me know your concerns. It will not lay when folded over or behind. Please refer to the operation video.

Out of the box only 1 hour and will not punch the holes the same distance from both left and right edge.Where is the actual manual for this item?

You have to move the side guide to get it even this punch is a piece of crap We won't we used it maybe two or three times then got rid of it.

What size combs to get when you run out?

Please do not worry about it, there is no limit to the size of combs.

Will it bind smaller size tablets

It binds any size - you just have to cut off the excess binding.
