Q: How often do they need to be replaced?

I got my first pair almost 25 years ago and I still have them. Of course I don't wear them every day because I bought several pair so I don't have to switch them but for the first couple of years I wore them every day. These insoles are very durable. I hand wash them and every 2 or 3 years I put them in boiling water and remold the arch. They work perfect for me and I hope that they never stop making them

Q: are these good for flat feet?

Better then most. Spenco rx comfort are completely flat

Q: Is this the thinsole version?

I don’t know what you mean by thinsole

Q: Do these provide heal pain relief ?

YES, for me they provide total relief for my plantar fasciitis heel pain, and allowed it to heal. I have been using them for over 15 years. Please make sure of your diagnosis.
