how well do these hold up?

I only had them for a few months and so far the hold up well. I did a lot of walking in them during a recent trip and they are very comfortable. The leather is scuff resistant.

This would be the width of the shoe C is narrower than D

What's up with the sole? Why do they wear down so quickly according to reviews?

softer sole faster wear, just like tires or any wear related item

Does anybody wear this for hallux rigidus?

I don’t have that, fortunately, but I’ll say these shoes are some of the most comfortable I’ve ever owned. Typical of Clarks. Perhaps these and a therapeutic insole would be sufficient.
