Is GFCI 50 amps (like picture) or 60 amps (like description)?

GFCI is 50 amps, not 60 amps. Hot tub supplier called for 60 amps, but I put in the 50 amp breaker anyway. When hot tub installer came, he said they always use 50 amp GFCI, so I was OK. They did however furnish a 60 amp feeder breaker for my main panel, but it was the wrong brand and I had to purchase a new feeder breaker that worked in my panel. Hope this helps.

Will this work on a 230v 50amp hot tub?

yes. but they sell 50 amp spa disconnects too

Have a 60amp 3 wire service, will this still work?

You really should have a 4-wire service so that everything is properly grounded. I am assuming you have 220 3-wire service now. One option is to install a new ground rod with the green wire connected to the green terminal - this would then give you 4-wire service. I recommend you contact a local electrician - this isn't something you want to be wrong about.
