Extra Virgin olive oil, Wheat Germ oil, White Oak Bark, Comfrey Root, Mullen Leaf, Black Walnut leaf, Marshmallow root, Gravel Root, Wormwood herb, Lobelia herb, Scullca?? (can't read, it's rubbed off) herb and beeswax.

Q: is it made from fresh or dried leaf?

99.9% likely that it is from dry leaf because if it were fresh, water would be introduced and the product would need a preservative as well as an emulsifier to not allow the oil and water-based leaf juice to separate.
