My 3 year old is 34 pounds and very tall with thin legs. The 3T on him are slightly too big in the leg, but will give him some growing room and not so tight that they leave a mark. Personally, I would pick the 12 month.

18 or 24! My son is the same size and 18 fit but are maybe too tight (or will be soon). I just ordered 24 to try now.

Q: Does this swim diaper prevent poop leaks as well as urine leaks?

Swimming diapers are basically meant to catch poop if it happens in the pool or on the beach. They're not like regular diapers so you will need to change the child right away but everything will be contained at least until you can get him or her changed.
