Q: How long will this adjust to? I've found my Neotech @ 21 inches is about an inch short for me with my tenor (but fine for my alto).

This strap is adjust from 17 to 21. You can try our ASIN B07ZFCVPX1 which is Adjustable from 22-27 inch, and will be more suitable for tenor sax.

Q: This comes with a plastic hook OR a metal hook and the customer doesn't get to choose?

Mine came with a plastic snap hook. Works well for me.
