Am I crazy, or did this double in price since a year ago?

If you seriously paid $235 for a $75 MSRP device, then sorry, but you're part of the current problem. It's thanks to people paying the 4 - 5x or higher prices that scalpers and scammers keep going. As for the 'there's none in stores', yes there are, but you have to be fast. Raspberry Pi has manufactured about 40,000 of these in the last year, but these people have been using bots to grab them all. Luckily now several US stores are implementing sales limits, or 'in store only' policies.

This must be the solid gold version?

Pretty much... Honestly tho, the hardware is solid but honestly, I'm no fan of these micro-hdmi cables, theyre really brittle so if you wanted to skip this one I wouldnt blame you.

I've purchased 2 of these and the fan does not come on. Is there a thermal switch or are these defective?

All I did was follow a youtube tutorial for Pi 4 specifically on where to plug in the fan on the pins. It worked immediately without a thermal switch. Assuming you've connected everything correctly with enough power source, if the fans are not blowing, I would guess a defect.
