Does this sheath have a leather welt down the middle to keep the edge of the axe from touching the metal rivets?

It does not. However, the rivet themselves compress the leather around them and in doing so keep the axe head from contacting the rivets. In the future if I notice the leather directly above the rivets starting to wear in a manner that would allow the axe head to actually contact the rivets I figured I could always install some type of liner to prevent contact.

what size blade will this fit?

It will fit a single bit standard axe. It fits perfectly on an axe I purchased from Home Depot which has a +/- 4-4.5" blade.
