Q: How do these work? Is the heel pad adhesive?

The straps come with a small heel sticker which has a small loop that you stick to the back of the shoe. Loop the strap through and they are perfect for the shoes you love, but prefer to have a straps for a better fit or look. However, I do want to warn you, the glue used is not strong. Whether you have a small ankle with ample slack to ensure the strap is loose, the sticker will come off midstride. I had to use gorilla glue for it to have a strong hold. Wore them multiple times after that and they are still going strong.
