Does this require power to work?

Yes it requires power. You should be able to use the same power cable you are using now to power the Fire stick. The power cable plugs in next to the Ethernet cable.

An Ethernet cable with a standard RJ45 connector. Cat5, Cat6 or Cat7 will work.

Will this unit achieve 480Mbps when paired with a 1Gbps modem and CAT 6 cable?

According to the description of the product on Amazon, the answer is "no". The Ethernet adapter described on the adapter is 10/100mbps which means that 100mbps transfer rate is the fastest speed the adapter is capable of regardless of whether its plugged into a GigE/Cat6 capable infrastructure. The Micro USB adapter on the unit may be capable of 480mbps max, but since the Ethernet adapter can only handle 100 mbps the data transfer rate of the micro USB is constrained by the throughput of the Ethernet adapter.
