Q: Can these hold 4x6 photos?

No they can't. I used them for my daughter's graduation wallet pics.

Q: Would these be appropriate to insert a cross stitch greeting? The cross stitch is on a piece of linen.

It may work. I just thought the insert area was very poorly made. Like when I tried to slide appropriately sized photo, the edges would rip away from the card.

Q: What is the actual opening dimension? Is the opening exactly 4.5x6.5”?

The opening is exactly 4.5 x 6.5 inches. I will add that it is somewhat difficult to fit the picture down into the card because of the tight fit on the sides, but I have been overall pleased with the product.
