Q: What are the dimensions of the sleeve?

5 in. by 5 1/2 in. Pocket for the CD is 5 inches. The extra 1/2 inch forms a tab above the opening where you insert the CD.

Q: Does anyone use labels with these? If so what are you using and how well do the labels stay on?

I use these for my 4000 plus DVD collection (all AFI top movies, half the Criterion Collection, top 100 Japanese, 70% of Roger Ebert top movies, etc). Two reasons: half the cost of the better quality sleeves, and thinner means I can get more into the 1000 MegaDisc storage cases. I am careful in handling my DVDs so these flimsy sleeves work ok. As for labels - I print the DVD info on an Avery return address label and place it at the top of each DVD sleeve. The labels do tend to peel some but none have fallen off completely after a few years. (Again, careful handling - the grandkids don't mess with these!).

Yes, they are double sided and you can fit 2 discs into one sleeve.

Q: noticed someone loved the first ones ordered but feel quality has changed. I want store cd and dvd~~ how can I find out the recent quality

Check where you are purchasing them from. Some stores sell a cheaper variant that has the same physical dimensions but use thinner materials. The ones I recently purchased (October 2017) from 1 Stop Outlet were of higher quality than the ones from another seller.
