Will this damage an interior hollow core door?

It will not.I have mine attached to a hollow core door. . . I'm a 250 pound male. No problems. It's a really smart design.

there is a 2inch step-down onto the floor of my room when you open the door...will this work with such a door from inside?

If your a person with long feet it might work. I wear a size 8.5 shoe and have the bar on the lowest position. Two inches higher a I don't think I could use it.

Should toe bar be mounted underneath bracket, as shown by other manufacturers of similar product, or on the top of the bracket as shown in picture?

I installed mines under the bracket.
