Q: Does this have a plasticy smell to it?

I have had it quite a bit so the initial smell has worn off but smelling it now I would say it is more of a slight rubbery smell.

Q: is this different to a [cheaper] pilates/yoga ball that needs inflating?

This one needs inflating too which is the whole point as sometimes a less inflated ball is required and verse vica. I don't have any experience of the cheaper pilates/yoga balls

Q: When this ball arrives in the mail is it deflated, and then you have to blow it up yourself?

Yes, the same as any yoga or Pilates ball.
